Ready to join The Nurse Source? To begin you need to complete your Application Form. The application file consists of Application Form, Nursing Skills List and Competency Tests. For your convenience we offer three simple options to do it. For your convenience, we offer three simple options for completing the application. - Option 1: You can complete the Application File on line.
- Option 2: You can download and print the forms. Then complete the printed forms and fax them to our office (Chicago or New York) or bring them with you when you come to our office for interview and orientation.
- Option 3: You can just come to our office Monday through Friday 9:00 a.m. to 3:00 p.m. And complete all necessary forms right there. Remember to bring all your credentials. No appointment required, just call us before getting on your way.
Application To begin the application process, please complete the form. Please answer every question truthfully and to the best of your knowledge. After you have submitted your online application, your profile will be saved in our database. You can then complete the appropriate Skills Checklists for your specialty or specialties. Download the Application Skills Checklist The information you provide by completing the Skills Checklist is utilized by The Nurse Source when we offer any assignments. We always strive to match your nursing skills and experience with the requirements for each open nursing job. Note: If you are experienced in several specialties listed below, please take the time to complete a Skills Checklist for each of these those specialties. Your additional experience allows you more assignment choices. Please complete the Skills Checklist and answer every question to the best of your knowledge. Competency Tests Competencies test verify your knowledge of various topics related to public safety, patient privacy, and infectious control and risk management. Annual compliance with these requirements a mandated by JCAHO and required by our clients. These tests need to be completed by every nurse a part of our application process.
